Important Notes
- No work may begin until written approval is received from the ACC.- Work not completed as approved may result in fines, legal action, and/or removal of the modification at Owner’s expense.
- Approval does not waive the need to comply with building codes, zoning laws, or other legal requirements.
BEFORE any work begins:
Submit ACC Modification Request to management
Work description + any plans/samples
Contractor license & Certificate of Insurance
Permits (if required)
Structural engineer report (if affecting walls/slab)
Flooring: soundproofing details if installing hard surfaces
Contractor Rules:
Work hours: Mon–Fri, 9 AM–5 PM
Sign in daily at the front desk
Use freight elevator/loading dock only
Protect & clean hallways; remove all debris (no chutes/dumpsters)
Approvals valid 6 months; finish work within 90 days
Unauthorized work = fines and possible removal at owner’s expense
Valid Certificate of Insurance on file
ACC Modification Checklist
This checklist is provided to help Unit Owners prepare and submit a complete ACC Modification Request. Incomplete applications will not be reviewed. Please confirm all items below are included prior to submission.
☐ Completed ACC Modification Request Form (signed by all Owners of record).
☐ Detailed description of proposed modification or alteration.
☐ Drawings, sketches, or plans showing location, dimensions, and materials.
☐ Color samples, material samples, or manufacturer specifications (if applicable).
☐ Structural Engineer’s report (required for modifications affecting load-bearing walls,
utilities, or structural integrity).
☐ Copies of all required permits or governmental approvals (if applicable).
☐ Contractor’s license and certificate of insurance naming the Association as an additional insured.
☐ Estimated start and completion dates (work must begin within 6 months of approval and be completed within 90 days).
☐ Owner acknowledgment of responsibility for ongoing maintenance, repair, replacement, and insurance of approved modification.
Special Flooring Rules
If replacing carpet with hard surface flooring (tile, wood, vinyl, marble, laminate, etc.):
- Submit flooring type and specifications
- Provide underlayment details (soundproofing)- Supply sound transmission test results (if required)- Show room layout and coverage
- Ensure at least 50% of each room (excluding kitchens/bathrooms) has rugs or carpet unless noise standards are met.
